1 - Register on Family ID

Please log onto the MVRHS website (www.mvrhs.org), Look for the Info for Families section on the right and click the link to "Family ID". 

 

The following things in Family ID need to be completed

 

ATHLETICS

This is where you register for your sport.  This is done for every sports season.

SPRING2017/2018

SPORTS REGISRTATION CLOSING DATE – 03/16/18

Family ID will re-open for late registrations - 03/19/18 at 6pm

 

Emergency Medical Information

This data includes medical information, and permission for over the counter medication administration.

The Emergency Medical Information must be completed once per school year.

 

Demographics

This data includes demographic information, parental contact information, permissions, and emergency contacts.

The Data Verification must be completed once per school year.

 

ImPACT Baseline Test

All 8th graders, freshmen, juniors and new students participating in MVRHS Athletics must take the ImPACT Baseline. 

Please register for a testing session on Family ID.  Pre-season testing sessions are available so that athletes do not miss practice time.

 

 

2 - Sports Physical

MIAA regulations require that athletes have a current sports physical while playing sports.  In order to be cleared, a sports physical must have been completed within the last 13 months and must indicate that the student is cleared to participate in sports or competitive athletics. The sports physical must remain current throughout the season.  If a sports physical expires mid-season, the student athlete will become ineligible.

 

The dates of the last sports physical on file with the school nurse may be found on Plus Portal under demographics. The sports physical must be turned in to the School Nurse or Athletic Trainer.
